USA Internship Program
Completing the Training/Internship Placement Plan for Trainees
The Training/Internship Placement Plan is an official government document that provides you, your trainee, CDS, and the U.S. government a detailed description of the training program. Please pay close attention to the guidelines below when completing it and answer the questions to the best of your ability.
Also included in the same download is the CDS Third-Party Agreement. It includes supplementary information required by the U.S. Department of State and an agreement verifying your understanding of the J-1 visa program.
After completing these documents, email them to your CDS representative. If you don’t have a CDS representative or have forgotten who your representative is, contact Gergana Hadzhiyska.

Section: Participant Information
- Name: Be sure the name is written as it is printed in the passport.
- Training/Internship Dates: These are the proposed dates of the training. Do not include travel time.
Section: Site of Activity Information
- Address: List the location where you intern will train
- Hours per Week: Trainees must work at least 32 hours per week.
- Stipend Information: Only include monetary compensation in this field. Do not include factors such as housing or transportation.
Section: Contract Agreement
- Supervisor Signature: The trainee’s supervisor must sign and date the form in the appropriate location. The Training/Internship Placement Plan is not valid without a signature.

Please fill in all sections using complete sentences. This document will be reviewed by a Customs and Border Patrol or consular official prior to your intern obtaining his/her visa, so it must give a clear picture of the intern’s responsibilities as well as the skills the intern will acquire during the internship.
CDS Third Party Agreement
Please complete and initial this document. Note that you will need to provide the following information:
- Employer Identification Number: this is your federal tax ID number.
- Dun & Bradstreet Number (DUNS): This number is no longer required by the US Department of State. If you do not have a DUNS number and would like to apply for one, you can register online at the Dun & Bradstreet website.
Additionally, if your state required you to have a workers’ compensation policy, you must present proof of it at this time.
